Iowa City woman forged doctor’s signature to fill prescriptions

IOWA CITY - An Iowa City woman faces multiple charges for forging a doctor's signature to fill prescriptions at Iowa City pharmacies.
According to Iowa City police criminal complaints, on Dec. 9 and 10, 21-year-old Ashley N. Shively presented prescriptions for promethazine with codeine and amoxicillin to Towncrest Pharmacy, Walgreens Pharmacy and CVS Pharmacy.
Police said the doctor whose signature was on the prescriptions had been forged. The doctor told police she did not write the prescriptions nor know Shively.
Johnson County Jail records indicate Shively is now in custody on two counts of fifth-degree fraudulent practice, two counts of identity theft, two counts of forgery, two counts of unauthorized use of a credit card and one count of prescription drug violation.
