1. Fill Out the Brochure
   • Complete the top box on the back of the informational brochure:
     • Italy’s Amalfi Coast & Rome
     • Canadian Rockies & Glacier National Park
     • The Wonders of Blue Danube
     • Best of Australia & New Zealand
2. Choose Your Add-Ons (Optional)
   • Trip Extension(s) (e.g., Florence for Italy, Prague for the Danube)
     • Just indicate if attending; no payment required at this time
   • Travel Protection Insurance (must be paid at time of deposit)
   • Cabin Selection (Danube only – see inside brochure for details)
   • Fill out the second box (Airport/Roommate Airport) – use CID
3. Information
   • Provide personal information (no passport needed at this stage):
     • Name
     • Phone Number
     • Address
     • Passport number and expiration date if applicable
4. Select Payment Method
   • Check:
     • Make payable to Premier World Discovery
     • Include it with the completed brochure
   • Credit Card:
     • Fill out the bottom box on the back of the brochure with complete and accurate card details
5. Return to The Gazette
   • Submit your completed brochure with payment
     • Drop off or mail in to
       The Gazette • Attn: Gazette Travel

   • We’ll forward everything to Premier World Discovery for processing.
