Taelyn Rae Holmes, 21 days old, of Cedar Rapids, passed away Friday, April 15, 2016. Memorial services: 10:30 a.m. Wednesday, April 20, at Stewart Baxter Funeral & Memorial Services, Cedar Rapids.
Survivors include her parents, Erin Holmes and Edward Jacobs; siblings, Krystal Bohannon, Jazmine Ross, Keeshawn Jacobs, Keeaira Jacobs and Logan Cassell; grandparents, Christine Holmes (Cody Wade), Terry Bradish (Michelle Frake) and Annie (Edward Mitchell) Jacobs; great-grandparents, Agnes Holmes, Suzanne Bradish and Zeola Mitchell; great-great-grandmother, Betty Warner; adoptive family of Amy Jo (Dan) Wolfs; and many aunts, uncles and cousins.
She was preceded in death by several great-grandparents; uncle, Anthony; and cousin, Wade McMahon.
