Formanek, Lori Kay
Lori Kay Formanek, 50, of Waterloo, formerly of Chelsea and Tama, died Saturday, June 8, 2013, at the University of Iowa Hospitals and Clinics following a short illness. Services will be at 10:30 a.m. Thursday, June 13, at St. Patrick's Catholic Church in Tama with Deacon Joe Behounek presiding. Interment will be in Maple Hill Cemetery, Gladbrook. Visitation will be from 5 to 8 p.m. Wednesday at the church. Arrangements are with the Hrabak Funeral Home in Belle Plaine.
Lori is survived by her daughter, Sterling Formanek of Chelsea; brothers, Calvin (Deb) of Mankato, Minn., Maynard (Pam) of Garwin, Marlin (Laurie) of Urbandale, Richard (Joan) of Piney Flatts, Tenn. and Pat (Diana) of Belle Plaine.
She was preceded in death by her parents, Harold and Bernice Thelen Wedmore.
