Charles “Chuck” A. Forman Jr., 82, passed away at Briarwood Health Care Center in Iowa City, Iowa. Funeral service Thursday, July 21, at 11 a.m. at Gay and Ciha Funeral and Cremation Services, 2720 Muscatine Ave., Iowa City. Visitation will be held on Wednesday, July 20, 4 to 7 p.m., also at the funeral home. Chuck will be laid to rest at Oak Hill Cemetery in Coralville, Iowa.
