Why channel difference in cable, satellite TV?

In Toledo, if you have cable television, you can get Des Moines, Waterloo and Cedar Rapids TV stations. If you have DirecTV (satellite), you can get only Cedar Rapids and Waterloo stations. My brother lives in Marshalltown and has DirecTV and gets only Des Moines stations. Why does the government let cable TV carry more local stations but not satellite TV?
