Doctor’s own choices left him unfulfilled

Regarding Dr. Normal Joshi's Feb. 27 column “Health model unfulfilling”:

Dr. Joshi is blaming the American medical system for not providing him with the “intrinsic motivation” that he needs. It appears to me that Joshi made some personal choices that resulted in his disenchantment.

First, he could have stayed in Nainital and gone into practice with his father, and possibly taken over the practice when his father retired. After deciding to come to the United States, he could have chosen to set up a practice in one of the thousands of small towns in America that are in desperate need of a doctor. And finally, he chose to become an officer in one of those large health care systems that stifled his personal satisfaction.
