Discounts needed if primary doctor not seen

Something to think about: You make an appointment with your doctor but see the physician's assistant. However, you and/or insurance provider are charged full price. In many cases seeing the physician's assistant is fine but why be charged the full price, why not receive a discount?
I recently asked this question when making an appointment at the University of Iowa Hospitals and Clinics. I was told the insurance company is billed under the staff doctor but my appointment is not with the staff doctor, or any other doctor, it's with the physician's assistant. Just think of the savings if discounts for such visits were reflected in the bills.
