Question over delay, impact of health bill

With all the controversy over health care change, two unanswered questions are stuck in my mind:
1. Why must a health insurance bill, that won't take effect for four years (after President Obama gets re-elected), be signed in the next two weeks or two months?
2. Why is a bill to help the 50 million uninsured going to have such significant effect on the 250 million who already have insurance?
