Jean K. Formanek, 97, of Keystone, passed away Monday, Dec. 22, 2014. A graveside service in memory of and to celebrate her life will be held at 2 p.m. Sunday, Aug. 16, at the Cedar Memorial Park Cemetery, 4200 First Ave. NE, Cedar Rapids.
Survivors include son, Dr. Robert V. Formanek Jr. and his wife, Janet, of Orinda, Calif.; daughter, Katha Ricciardi and her husband, Vezio, of St. John, Virgin Islands; and two grandchildren, Kara E. Formanek of Alameda, Calif., and John R. Formanek of San Francisco, Calif.
