Judy Ann Formanek, 74, of Chelsea, passed away with her three children by her side following a sudden illness on Saturday, May 28, 2016, at the University of Iowa Hospitals and Clinics. Judy Ann was born Aug. 28, 1941, on the family farm in rural Tama County, the daughter of Raymond and Leona (Kucera) Bennett. She graduated in 1959 from Chelsea High School. On Feb. 27, 1960, Judy married Richard Emil Formanek at St. Michael Church in Belle Plaine. Together the couple bought her family's farm in 1961, where she had lived to the current date. She worked as a farm wife, a housekeeper at the casino and a nursing home in Toledo.
Judy is survived by her children, Steven (Kelly Jo) Formanek of Belle Plaine, Rob Formanek of Chelsea and Kelly (Dennis) Deke of Traer; three brothers, Gene, Gary "Guy" and Eldon "Osh" Bennett; grandchildren, Richard Dean Formanek, Shawn (Britney) Formanek, Sterling Kay Schmitt (Alex) and Kalan Richard Deke; great-grandchildren, Brianna Griffith, Alexys, Zachary and Logan Formanek and Savy Kay Schmitt; and many more loving family and friends.
She was preceded in death by her parents; husband, Richard Emil Formanek, (1982); and sister, Janice Bartachek.
Judy enjoyed maintaining her farm, gardening, flowers and spending time with her grandchildren and great-grandchildren.
