RICHARD "DICK" CHIZEK

Richard Lee "Dick" Chizek, 77, of Toledo, passed away Saturday, May 9, 2015, at Covenant Medical Center, Waterloo. Memorial Mass will be 10:30 a.m. Thursday, May 14, at St. Patrick Catholic Church in Tama, with Father Michael Mescher as celebrant. Interment will be at Buckingham Cemetery in Traer at 3 p.m. Thursday, following a luncheon at the church. A memorial visitation will be Wednesday, May 13, from 5 to 7 p.m. at Kruse-Phillips Funeral Home in Tama.
He is survived by his significant other, Cindy Kearney of Toledo; his wife, Alice Chizek of Traer; one son, David Welby (Lisa) Chizek of Toledo; one daughter, Julie Green of Traer; his grand-children, Malynda (Julien) Chizek Frouard, Daniel (Angel) Chizek, Nicholas Chizek, Kelly Chizek, Jesse (Bryana) Chizek and Kacey (Stephen) Johnston; five great-grandchildren, Mason, Alexis, Hunter, Melody and Robert; and his half sister, Shirley (Ronald) DeBondt of Grinnell.
Preceding him in death were his parents; his stepmother, Elsie Kos Chizek; one daughter, Rebecca, in 1982; and a stepbrother, Wayne Kos.
